Shoulder Pain & Rotator Cuff Treatment in Kolkata
Physiotherapy for rotator cuff problems, shoulder impingement and pain when lifting the arm

Overview
Shoulder pain that comes on when you lift your arm, reach behind your back or lie on that side is very common. It is most often caused by the rotator cuff, the group of muscles and tendons that controls the shoulder, and is sometimes called impingement. Most people improve with a well-planned exercise programme and changes to daily activities. Every first visit starts with an assessment by Dr. Partha Sarathi Neogi to find out what is causing your pain. If your shoulder has become very stiff in all directions, our frozen shoulder page may be more relevant.
How Dr. Neogi assesses shoulder pain
- Your history: how the pain started, which movements hurt, night pain, your work and sport, and any injury or fall.
- Examination of shoulder movement and strength, rotator cuff tests, and the neck and upper back, which can also cause shoulder pain.
- Checks for signs of a significant tear, dislocation, fracture or a problem that needs a doctor first.
- A plain-language explanation and a plan, including exercises for home.
Rotator cuff pain and impingement, and how many sessions you may need
The rotator cuff keeps the ball of the shoulder centred while you move your arm. Its tendons can become painful when they are loaded more than they can handle, for example by a sudden increase in overhead work, gym training or sport, or gradually with age. Changes seen on scans, including small tears, are common in people without pain, so treatment is guided by your symptoms and how your shoulder moves.
Care is mainly progressive strengthening of the rotator cuff and shoulder blade muscles, with advice on which activities to reduce for a while. Joint mobilisation, soft-tissue work and, in some cases, dry needling or therapeutic ultrasound can ease pain alongside exercise. A large tear after an injury, or pain that does not improve, may need an orthopaedic opinion. How many sessions you need depends on the cause and how you respond, and progress is reviewed at each visit.

What you can do at home
- Keep the shoulder moving within comfort; avoid holding it still for long periods.
- Reduce heavy or repeated overhead tasks for a while, then build back up.
- Sleep on the other side or on your back, with a pillow supporting the painful arm.
- Do your exercises regularly; mild discomfort that settles within a day is acceptable.
- For pain medicine, follow your doctor's or pharmacist's advice.

Frequently Asked Questions
The rotator cuff is a group of four muscles and tendons that control the shoulder. When the tendons are overloaded they can become painful, especially when lifting the arm. Most rotator cuff problems improve with exercise.
It is a term for pain when lifting the arm, often felt at the side of the shoulder. Today it is usually treated as a rotator cuff problem, with strengthening exercises rather than rest.
Most people do not need one at first. An MRI or ultrasound scan is useful after a significant injury, if a large tear is suspected, or if you are not improving.
In frozen shoulder the shoulder becomes stiff in all directions, even when someone else tries to move it. In rotator cuff problems movement is usually possible but painful. An assessment tells them apart.
Many tears, especially those that come on gradually, are managed well with exercise. A large tear after an injury may need an orthopaedic surgeon's opinion.
It depends on your condition. After the assessment you will be told what to expect, and your progress is reviewed at each visit.
